Abstract

The objective of this study is to demonstrate the importance of physician–pharmacist interconsultation in the evaluation of an older patient with polypharmacy at risk of hyponatremia through the analysis of drug interactions, with a focus on preventing hospital admissions. Additionally, the study aims to estimate the costs avoided in the management of mild hyponatremia in a polymedicated older patient treated at a Primary Health Care (PHC) clinic of a health plan operator, compared with the costs associated with hospitalization for hyponatremia in a hospital within the accredited provider network.
